Caring Caller: Case Studies

Here are just a handful of case studies detailing how our Caring Caller has made a difference to local communities:

Case Study #1:

Discharge crew reported they were unable to complete a discharge of a patient due to a family member not being able to support the patient on their own, without a care package in place. The crew noted the care package was due to start the following day.

Arrangements were made to for the patient to return to hospital overnight and, the following day, our Caring Caller discussed their concerns with the crew who were allocated to take the patient home again and advised them of the previous concerns.

When the crew took the patient home, confirmation was received that the care package was in place and the family were happy to receive the patient into their care.

Case Study #2:

Discharge crew took a 93-year-old female patient home from hospital and noted her mobility was poor and she was very cautious when moving. The crew were informed the patient had been hospitalised for a prior fall.

The patient lived with her husband who was sadly in hospital at the time following a stroke. This meant the lady would be home, alone. It was also found that no care package was currently in place.

The crew discussed their concerns with the patient and she agreed she may benefit from help at home. Our Caring Caller was then able to make a referral to the Independent Living Service to request a care assessment and an OT assessment.
